ANDROGENIC ALOPECIA OR BALDNESS COMMON

 

Androgenic alopecia (or common baldness) is a chronic, genetically predetermined condition characterized by progressive involution into miniaturization of the hair follicles of the scalp and hair that comes from it.
Clinically it comes (according to James Hamilton, who in the 1950s classified the types of baldness) from a progressive retreat of the hair insertion line, from the opening of the frontoparietal angles (stemming) that gives the frontal line (hairline) the characteristic male M-shape, from the slow hair loss of the vertex, up to the alopecic involvement of the entire upper part of the scalp with typical savings of the nape and temporal zones above the earphones, to finally arrive at the "crown baldness".
Androgenetic alopecia is accompanied often, but not constantly, by seborrhea and furfure peeling. In men hair loss begins after puberty. The first change consists, as a rule, in a biparietal frontal recession, present in 96% of sexually mature Caucasian males, even in those in whom hair loss is not destined to progress (physiological male frontoparietal alopecia).
The frontoparietal recession, therefore, can depend on mechanisms different from those of typical baldness of the vertex. The classic "androgenics" begins around the age of 17-18; the fall is more or less constant, usually never high quantitatively. Hair follicles have a well-determined life cycle:

 

- Anagen phase: growth
- Catagen phase: transition
- Telogen phase: rest

However, the problem of androgenetic outflow in telogen does not arise from the quantity of hair falling out, but from the fact that many of them are progressively replaced by shorter, thinner, less deeper elements. , which are the prelude to the final disappearance of the hair itself.
James Hamilton, in the 1950s, classified the types of baldness based on the degree of frontoparietal, frontal recession and thinning of the vertex. O'Tar Norwood, more than 25 years later, perfected and completed this "classification for illustrations" of androgenic alopecia.
According to Hamilton (1951), 5 stages can be distinguished. Norwood modified Hamilton's classification (1975), proposing a "ladder" of 7 stages, some of which are further divided to obtain 12 possibilities.

OTHER TYPES OF ALOPECIA

 

ALOPECIA TELOGEN EFFLUVIUM: hair falls out in large quantities without forming any stains;

CAUSES

 

The causes can be many: hereditary factors, hormonal alteration, medication intake, particular vitamin deficiencies (such as lack of iron), or bad life and food habits. 5-alpha-reductase, is an enzyme system that converts testosterone to dihydrotestosterone, which is considered as one of the fundamental factors in the onset of alopecia.

 

CICATRICIAL ALOPECIA

 

It is a definitive form that comes from autoimmune causes with disappearance of the follicle and hair papilla (neoplasms, follicular mucinosis, aplasia cutis verticis, scleroderma, lupus, lichen planus, folliculitis).

 

PSEUDOALOPECIA

 

Form arising from traumatic, chemical, infectious or congenital anomalies.

POST-TRANSPLANT TRICOPIGMENTATION

 

Technique FUT (Follicular Unit Transplantation) or FUSS.
It is a technique of autotransplantation of hair that consists in the removal of a strip of scalp (STRIP) from the donor area that is sutured.
